- Abstract
A obstrução esofágica é uma patologia comum em equinos, tendo como causa primária geralmente a ingestão de material impróprio ou conteúdo alimentar. Grande parte dos casos de obstrução é resolvida com tratamento clínico;no entanto, quanto maior o tempo de obstrução, maiores as chances de complicações, como a ulceração de mucosa, fístula e formação de divertículo. O divertículo é uma falha na musculatura do esôfago resultante de trauma e consequente retração cicatricial, com redução ou impedimento peristáltico. O objetivo desse trabalho é relatar o caso de uma égua, Quarto de Milha, 400 kg, 4 anos de idade, atendida no Hospital Veterinário da FZEA-USP, com obstrução esofágica e histórico de obstruções recorrentes. No atendimento clínico constatou-se taquicardia, taquipneia, tosse, sialorréia, secreção marrom bilateral, estertor respiratório, temperatura corporal em 36,9 °C, mucosas normocoradas e TPC 2, hematócrito 40%, proteína plasmática 6,8 e leucocitose. Realizou-se sondagem nasogástrica após aplicação de 0,3 mg/kg de escopolamina intravenosa diluída em 1 litro de ringer lactato, porém a sonda não progredia do ponto de transição para a região torácica do esôfago. Na tentativa de desobstrução foram administrados 20 ml de lidocaína 2% injetável via sonda e, em seguida, 2 litros de água morna sem pressão. No primeiro momento houve o retorno da água com conteúdo alimentar; em seguida houve liberação da passagem pelo esôfago sem progressão da sonda nasogástrica, indicando que a obstrução havia se desfeito. Após este procedimento o animal apresentou-se mais calmo, com diminuição da salivação e dispneia, porém ainda com tosses esporádicas. Instituiu-se tratamento medicamentoso com flunixin meglumine [1,1 mgkg, SID,IV] por 5 dias, ceftiofur [4,4 mgkg, SID, IV] por 7 dias, gentamicina [6,6 mgkg, SID, IV] e DMSO [0,5 mgkg, BID,IV] durante 3 dias. Posteriormente foram agendados novos exames no Centro de Odontologia Equina da USP, onde na radiografia contrastada com sulfato de bário foi possível visualizar divertículo esofágico na região de transição de cervical para torácico. Na esofagoscopia visualizou-se área de dilatação esofágica compatível com divertículo, motilidade incompleta e acúmulo de líquido e de fibras vegetais. Após 24 horas de tratamento o animal apresentou melhora significativa, deglutindo água e alimentando-se com pequenas quantidades de capim fresco a cada duas horas. O aumento da quantidade oferecida foi gradativo. O animal recebeu alta três dias após a internação, com mudanças no manejo alimentar. Prescreveu-se dieta à base de capim coast cross por 20 dias seguidos de introdução ao feno, sempre mantido acima da base da cabeça, e a ração peletizada fornecida em quatro refeições diárias. O animal não se adaptou à ração peletizada, apresentando crises de disfagia e tosse após ingerir a ração, porém sem problemas na alimentação à base de volumoso. No caso em questão foi importante estabelecer um diagnóstico para a introdução de manejo alimentar adaptado que, junto à terapia medicamentosa, foi essencial para manter o animal estável e sem novas obstruções, já que a lesão tecidual esofágica que levou à formação do divertículo é de difícil reversão.
