- Abstract
A 71-year-old man was admitted to our hospital with muscle pain on legs, anorexia, vomiting and fever after the discontinuation of oral betamethasone. Laboratory studies revealed elevated CRP, and acute adrenal insufficiency. We diagnosed atypical familial Mediterranean fever (FMF) based on the clinical manifestations and a genetic analysis revealing multiple heterozygous mutations in exon 2 of the MEFV gene (L110P/E148Q). The administration of oral daily colchicine (0.5 mg/day) resolved the clinical and laboratory findings. This case suggests that genetic factors such as heterozygous mutations of MEFV gene and environmental factors such as steroid withdrawal are involved in the onset of FMF.
