- Abstract
Compared to the uterine tumour resembling ovarian sex cord tumour (UTROSCT), which traditionally exhibits >50% sex cord elements and a relatively benign course, endometrial stromal tumour with sex cord like elements (ESTSCLE) have been defined as <50% sex cord elements with the presence of a JAZF1 fusion and are associated with worse outcomes. We present a rare case of an ESTSCLE, which was originally misinterpreted as carcinosarcoma on biopsy, showing >50% sex cord elements, poor clinical behaviour, equivocal immunohistochemical (IHC) features and a PHF1 unbalanced gene rearrangement. We will address common diagnostic pitfalls of uterine mesenchymal tumours with sex cord histology as well as the challenges faced when differentiating between UTROSCT and ESTSCLE.
