Albuminurie: ein kardiovaskulärer Risikoindikator.

Zusammenfassung Hintergrund: Der �bertritt von Albumin aus dem Gef��lumen in das umliegende Gewebe stellt immer eine ernsthafte St�rung der Integrit�t des gesamten Gef��systems dar. Klinisch werden diese Ver�nderungen als „cotton-wool“-Herde in der Retina oder durch das Auftreten von Albumin im Urin erkennbar. Dabei steigt mit der H�he der Albuminausscheidung das kardiovaskul�re Risiko auch schon unterhalb der Schwellenwerte f�r Mikroalbuminurie an. Pathophysiologie und Definitionen: Ein �bertritt von Albumin in das Gewebe stellt einen Hinweis auf eine St�rung der Schrankenfunktion der Endothelzellen dar. In der Niere kommt es u.a. zu einer Sch�digung von Podozyten, Mesangium- und Endothelzellen, einem Verlust der Ladungsselektivit�t und einer ver�nderten Expression von Matrixproteinen. Diese Ver�nderungen sind aber nicht nur in den Nieren, sondern auch im Myokard nachweisbar. Die definierten Grenzen f�r die Mikroalbuminurie (> 30 mg/24 h) und Proteinurie (> 300 mg/24 h) sind f�r die kardiovaskul�re Prognose jedoch nur eingeschr�nkt bedeutsam, da die Risikoerh�hung bereits bei einer geringeren Albuminausscheidung beginnt. H�ufigkeit und Bedeutung: Die Pr�valenz einer Mikroalbuminurie liegt in der Bev�lkerung bei etwa 8%, in Hochrisikogruppen dagegen bei bis zu 50% und mehr. Sie ist mit einer Erh�hung der kardiovaskul�ren Morbidit�t und Mortalit�t verbunden. Therapieoptionen: Eine Reihe von therapeutischen Ans�tzen (enge Blutzuckereinstellung, Blutdrucksenkung, Lipidsenker) f�hrt sowohl zu einer Verminderung der Albuminurie als auch zu einer Verbesserung der kardiovaskul�ren Prognose. Als zentraler Angriffspunkt der kardiovaskul�ren Protektiongilt die wirksame Hemmung des Renin-Angiotensin-Aldosteron-Systems (RAAS). RAAS-blockierende Substanzen sind nicht nur mit einer Verringerung von Endorgansch�den (Herzinsuffizienz, diabetische Nephropathie, zerebrovaskul�re Ereignisse) verbunden, sondern k�nnen auch zu einer Verminderung der Mortalit�t f�hren. Praxisempfehlung: Die fr�hzeitige Diagnose, eine gr�ndliche kardiovaskul�re Diagnostik und engmaschige Einstellung der kardiovaskul�ren Risikofaktoren unter konsequentem Einsatz von RAAS-blockierenden Substanzen erscheinen bei Patienten mit dem Nachweis einer Albuminurie angezeigt.
