- Abstract
The article discusses the Practice Forum which aims to provide social work practitioners with information on the ambulatory integration of the medical and social (AIMS) model. AIMS was developed at Rush University Medical Center (RUMC) in consultation with a panel of experts in primary care models whose goal is to integrate medical and nonmedical needs. A retrospective evaluation indicated that AIMS patients had less hospital admissions and emergency department visits.
