Liposomal Amphotericin B as Initial Therapy for Invasive Mold Infection: A Randomized Trial Comparing a High-Loading Dose Regimen with Standard Dosing (AmBiLoad Trial).

- Abstract
El estudio de Herbrecht et al. confirmó la superioridad de voriconazol sobre anfotericina convencional en el tratamiento de la aspergilosis invasiva1. Sin embargo, las frecuentes interacciones farmacológicas de los azoles y la posibilidad de conseguir dosis elevadas (hasta 10 mg/kg/día), bien toleradas con anfotericina B liposomal, hacían prever un importante papel de anfotericina B liposomal en el manejo de esta patología infecciosa. El estudio comparó de forma prospectiva y aleatorizada dos dosis de anfotericina B liposomal (3 vs. 10 mg/kg/día) durante 14 días, seguidos de 3 mg/kg/día, en primera línea de aspergilosis invasiva. El objetivo primario analizaba la respuesta (completa o parcial) de ambas estrategias al fin del estudio, hasta la secuenciación a 3 mg/kg/día. Se aleatorizaron 201 pacientes, fundamentalmente oncohematológicos (93%), con un porcentaje de neutropénicos (<500/mm³) del 73%. Se confirmó una respuesta favorable en 50% de los que recibieron 3 mg/kg/día, respecto a un 46% de los que recibieron 10 mg/kg/día (diferencia 4%; IC95% de la diferencia: -10% a 18%; p >0,05). Tampoco hubo diferencias en la supervivencia a 12 semanas (72% vs 59%; p>0,05) (objetivo secundario), pero sí se documentó una significativa mayor diferencia de nefrotoxicidad e hipopotasemia en los pacientes que recibieron 10 mg/kg/día. Los pacientes con ausencia de control del proceso oncológico y los sometidos a trasplante alogénico se asociaron con peor supervivencia en el análisis multivariante. El estudio concluía que anfotericina B liposomal a dosis de 3 mg/kg/d obtenía unos resultados clínicos satisfactorios en el tratamiento de la aspergilosis invasiva en pacientes oncohematológicos y con menor toxicidad que la dosis de 10 mg/kg/día.
