Flank or midline ovariohysterectomy in the cat?

- Abstract
Both the left flank and the midline approach may be used for ovariohysterectomy in the cat, and either approach may be appropriate in most cases. However, recent research suggests that the midline approach may be less painful, and therefore should be considered more often. Definite advantages of the midline approach include improved exteriorisation of the right ovary and of the gravid uterus, particularly in late pregnancy, but the risks of wound contamination and of dehiscence must be remembered, particularly in feral cats. It also avoids the coat colour changes seen if the flank is shaved in Siamese or Burmese cats. The left flank approach sometimes makes elevation of the right ovary or exteriorisation of the uterine bifurcation problematic. It is less useful in the heavily pregnant animal, in which a large flank incision would be needed and would cause more muscle damage. However, it is preferable in lactating individuals, may be preferred in the management of feline fibroadenomatous mammary hyperplasia, and has some advantages in stray cat spay projects.
