- Abstract
This article discusses the issue of fuel poverty and its impact on health. It highlights the high number of deaths attributed to cold in England and Wales and recommends the commissioning of a single point of contact to support vulnerable individuals. The Nottingham Energy Partnership (NEP) is mentioned as a resource for accessing support, but referrals from healthcare clinicians have been below capacity. The article then describes a Population Health Management (PHM) project in Nottingham and Nottinghamshire that uses data to identify at-risk patients and generate referrals for support. The project has been successful in providing various interventions to referred patients, including insulation and energy-saving measures. The article concludes by suggesting that this PHM approach could be applied more widely to address health inequality and fuel poverty.
