Multifactorial analysis of early pregnancy loss following an IVF programme.

- Abstract
Objective: To analyse and reveal some relative factors of miscarriage post IVF-embryo transfer. Materials/Methods: Two-year cases of IVF and embryo transfer treatment were analysed. Differential expression profile was obtained using microarray by comparing the normal deciduas and the deciduas from aborted patients by sample pooling and RT-PCR, northern blot, in-situ hybridization were used to prove this result. Results: During the 2-year period, the overall clinical pregnancy rate was 40.9% (1421/3474) of which pregnancy rate was 43.2% (811/1877) of fresh IVF-embryo transfers, 39.2% (344/877) of fresh intracytoplasmic sperm injection (ICSI)-embryo transfers and 36.9% (266/720) of frozen embryo transfer. The incidence of clinical pregnancy loss was 5.9% in fresh IVF-embryo transfer, 10.5% in fresh ICSIembryo transfer and 10.2% in frozen embryo transfer. The incidence of early pregnancy loss was higher after ICSI and frozen embryo transfer than after routine IVF, but there was no significant difference in the incidence of abnormal karyotype among these groups. During weeks 5 to 8 post embryo transfer (PET), yolk sac could be detected in all ongoing pregnancies (group A), while 71.6% in poor pregnancy outcome group (group B). During weeks 5 to 10 PET, all pregnancies displayed heart beat motion in group A, while 39.2% in group B. Microarray investigations revealed 212 up-regulated and 156 down-regulated genes in aborted deciduas comparing to normal deciduas. Chemokine (C--C motif) ligand 21 (CCL21) and indoleamine-pyrrole 2,3dioxygenase (IDO) displayed decreased mRNA expression in the aborted patients, while FGG and IL-8 displayed a little increase in mRNA expression both by RT-PCR and northern blot. Although individual difference was observed among different samples, the general trend was similar. Conclusion: Multiple factors may involve in the aetiology of miscarriage. Yolk sac and heart beat can reflect partially the fetal state. The aberrant expression of some genes may be related to miscarriage.
