MÚLTIPLAS ALTERAÇÕES CELULARES EM UMA EFUSÃO PLEURAL NEOPLÁSICA.

- Abstract
A avaliação citológica do derrame pleural é uma forma simples de se obter um diagnóstico de derrame neoplásico que é uma complicação rotineira na clínica de pacientes oncológicos e pode significar disseminação ou progressão da doença. Na fotomicrografia, observa-se a citologia do líquido pleural de uma cadela SRD, 10 anos, com histórico de realização de mastectomia em bloco, um ano antes, devido a presença de nódulo ulcerado em mama abdominal cranial direita. No dia da consulta, a cadela apresentava um quadro de angústia respiratória. Foi realizado exame ultrassonográfico, o qual revelou a presença de líquido livre na cavidade torácica. O material foi drenado e encaminhado para análise. Na avaliação citológica do líquido, observou-se alta celularidade, com contagem total de células nucleadas 12.400 (IMAGEM A), composta predominantemente por células epiteliais individualizadas e por vezes em arranjos moruliformes (setas vermelhas). Muitas células possuíam grandes vacúolos citoplasmáticos, deslocando o núcleo excentricamente, anel em sinete (setas pretas). Anisocitose e anisocariose. Presença de neutrófilos íntegros e picnóticos, e fundo de lâmina com algumas hemácias. Verificou-se também, emperipolese (IMAGEM B- seta no canto superior direito), mitoses atípicas (IMAGEM C- seta canto superior esquerdo), e típicas, assim como a presença de células multinucleadas (IMAGEM D), por vezes com amoldamento nuclear (seta azul) e canibalismo celular (seta verde). Considerando essas características citológicas, a efusão foi classificada como efusão neoplásica. Além da presença simultânea dos múltiplos critérios de malignidade como o canibalismo, emperipolese e o anel de sinete, a conformação do arranjo das células epiteliais também corroboram com a hipótese de que essa efusão é uma complicação resultante da disseminação da neoplasia primária. Esses achados reforçam a importância da análise citológica de derrames cavitários em pacientes oncológicos, devendo sempre ser realizada por especialista, para que o exame contribua na compreensão do quadro clínico e abordagem terapêutica do paciente.
