- Abstract
Si bien la necesidad de resolver un problema clínico lleva a establecer un punto de partida para abordarlo (estudio de riesgo, pronóstico o tratamiento), en todos los casos se busca atribuir causalidad. El razonamiento clínico desglosado en el libro Clinical Epidemiology. The architecture of clinical research ofrece una guía sencilla para entender este fenómeno y utiliza tres componentes: estado basal, maniobra y desenlace. En este modelo se describen distintos errores sistemáticos (sesgos) que pueden ocurrir al soslayar características de esos tres componentes básicos. Las omisiones de características en el estado basal provocan un ensamble inadecuado de la población y el sesgo de susceptibilidad; en la aplicación o evaluación de la maniobra, el sesgo de ejecución; y en la evaluación del desenlace, el sesgo de detección y el sesgo de transferencia. Así, es importante hacer énfasis en que si esta forma de razonamiento facilita el entendimiento del fenómeno causal, las variables por seleccionar en los estudios a las que se les atribuirá o no causalidad requieren razonamientos clínicos adicionales que evalúen su pertinencia.
