The article presents a case study of a 26-year-old woman having new-onset diabetic ketoacidosis. Transthoracic echocardiogram showed right ventricular volume overload and agitated saline revealed an intracardiac shunt. Cardiac resonance imaging affirmed intra-atrial septum discontinuity which is representative of atrial septal defect's (ASD) secundum.
