Anormal Uterin Kanamalı Kadınlarda Pipelle ile Alınan Endometrial Biyopsi Sonuçları ile Histerektomi Patoloji Sonuçları Ne Kadar Uyumlu?

- Abstract
Aim: To compare the preoperative results of endometrial sampling taken with pipelle and the results of pathology specimens examined after hysterectomy in patients who are planned to have hysterectomy with Abnormal Uterine Bleeding (AUB), and to examine the accuracy of biopsy in the diagnosis. Material and Method: In this retrospective study, the records of the patients who referred to our clinic with AUB between January 2014 and January 2018, who underwent endometrial biopsy with pipelle and then had hysterectomy were investigated. The demographic data of the patients were recorded, and the histopathological results of the endometrial biopsies and hysterectomies were received. The diagnostic accuracy, sensitivity, specificity, and positive and negative predictive values were calculated. Results: A total of 387 patients were included in the present study. The mean age of the patients was 46.6±6.2 years. With pipelle endometrial biopsy, the highest sensitivity in the final pathology in hysterectomy materials was endometrium cancer (33.3%), simple endometrial hyperplasia with atypia was the highest specificity (99.1%), simple endometrial hyperplasia without atypia was the highest positive predictive value (42%), complex endometrial hyperplasia with atypia was with the highest negative predictive value (97.6%), and complex endometrial hyperplasia without atypia was with the highest diagnostic accuracy (95.8%). Conclusion: Endometrial biopsy is a sensitive and specific test in the diagnosis of endometrial pathology, but additional diagnostic methods may also be necessary in focal lesions. In this study, the accuracy of the diagnosis was found to be at the highest level in the complex endometrial hyperplasia and endometrial cancer.
