The article presents the text of speech by Doctor Kirby I. Bland, 2008-2009 president of the Southeastern Surgical Congress, delivered at the Annual Scientific Meeting and Postgraduate Course Program in Atlanta, Georgia from February 7-10, 2009, in which he discussed the rewards, threats and challenges in the practice of general surgery, the factors influencing the surgical career and the medical school curriculum.
