RETROSPEKTIVNÍ ANALÝZA CHEMONEUROLÝZY JAKO SYMPTOMATICKÉ LÉČBY BOLESTI U ONKOLOGICKÝCH PACIENTŮ.

- Abstract
Cíl: Cílem studie je zjistit naše indikace pro chemoneurolýzu pod CT kontrolou jako paliativní léčbu pro zmírnění bolesti u pacientů s nádorovým onemocněním a identifikovat skupiny pacientů, kteří mají z této intervence největší prospěch, pokud jde o léčbu bolesti. Metodika: V našem retrospektivním přehledu bylo v letech 2013-2023 hodnoceno 27 zákroků u 25 pacientů. Zákroky byly prováděny z dorzálního přístupu pod CT kontrolou, neurolýzy bylo dosaženo v lokální anestezii (10-20 ml Bupivacainu) instilací 10 ml 96% etanolu. Nejčastěji byla prováděna neurolýza coeliackého plexu (19 výkonů) u 15 pacientů s diseminovaným karcinomem pankreatu. U dvou pacientů byla neurolýza pro dobrý klinický efekt opakována. Další dvě indikace byly cholangiocelulární karcinom a karcinom žaludku. Provedli jsme sedm neurolýz ganglion impar (šest pacientů s karcinomem rekta, jeden pacient s karcinomem perinea) a jednoho horního hypogastrického ganglia (jedna pacientka s karcinomem vulvy). Pooperační komplikace a klinický výsledek byly sledovány 1. a 30. den po propuštění Centrem paliativní medicíny a léčby bolesti. Výsledky: Všechny zákroky byly technicky úspěšné a nevyskytly se žádné komplikace po zákroku. Průměrná doba přežití po výkonu byla 104 dní, bez významného statistického rozdílu mezi muži (83 dní) a ženami (120 dní) (p = 0,92). Úlevy od bolesti bylo dosaženo u 22 z 27 zákroků (81 %), snížení na stupnici VAS o 1-4. V pěti případech (25 %) však pacienti nezaznamenali dostatečnou úlevu od bolesti. Závěr: CT navigovaná etanolová chemoneurolýza je rychlý, bezpečný a levný postup, v našem souboru bez významných komplikací a s dobrou účinností.
