Tratamento conservativo de fratura proximal cominutiva infeccionada de osso metatarsiano II.

- Abstract
Fraturas dos ossos metacarpianos ou metatarsianos acessórios são comumente observados na clínica de equinos em animais de diferentes idades. Além disso, lesões nesses ossos são frequentemente causas de claudicação e de danos secundários a estruturas adjacentes, como o ligamento suspensor ou articulações proximais. As principais causas das fraturas são traumas diretos na região, mas também podem ocorrer de forma espontânea durante os exercícios. As fraturas podem ainda se apresentar fechadas ou expostas, simples ou cominutivas, e sua localização pode variar de acordo com a região acometida (proximal, média ou distal). As fraturas ósseas da porção proximal são as fraturas mais complexas e a intervenção cirúrgica é complicada pela necessidade de fornecer estabilização do fragmento residual. A claudicação e o inchaço geralmente são evidentes, mas melhoram rapidamente com medidas de primeiros socorros. A ocorrência de fraturas cominutivas é frequente e as feridas cutâneas muitas vezes se comunicam com o foco de fratura. Foi solicitado atendimento de uma égua Quarto de Milha, 10 anos, que apresentava histórico de coice na região medial da canela havia 20 dias, com uma ferida que não cicatrizava no local e claudicação do membro acometido. O animal apresentava claudicação grau 4 do membro pélvico esquerdo, aumento de temperatura, sensibilidade dolorosa, edema e a ferida na região medial com bastante secreção purulenta. Após avaliação clínica, realizou-se exame radiológico do local, constatando fratura cominutiva de II metatarsiano, com sinais de osteomielite. Na avaliação hematológica, notou-se leucocitose por neutrofilia. Como tratamento, optou-se pela limpeza da ferida com iodo degermante, bandagem compressiva, antibiótico sistêmico ceftiofur (4,4,mg/Kg, SID, 10 dias), perfusão regional com amicacina (1 mg a cada 48 horas, 10 dias) e fenilbutazona (4,4 mg/Kg, SID, 3 dias), seguidos por 40 dias de firocoxibe (0,1 mg/Kg, SID) e suplementação mineral à base de cálcio durante 90 dias. Realizouse acompanhamento clínico e radiológico do animal semanalmente, sendo estabelecido protocolo de retorno às atividades após controle local da infecção: caminhadas controladas, seguidas de trote guiado por 5 min após 7 dias, aumentado a intensidade dos exercícios semanais até o retorno da atividade montada aos 60 dias após o início do tratamento. Aos 90 dias, após remodelamento ósseo no local e melhora clínica da claudicação, o animal retornou às atividades esportivas sem sinais de claudicação. Apesar do tempo de evolução, foi possível obter um bom resultado com relação à cicatrização óssea a partir do controle da infecção no foco de fratura, associando antibioticoterapia sistêmica e local. Além disso, o retorno precoce e controlado de atividade física contribuíram para a recuperação do paciente durante a fase de cicatrização da lesão, sem complicações.
