- Abstract
There are several diagnostic procedures and tools available to investigate lameness originating from the fetlock region in horses. The evaluation should start with a thorough clinical examination and gait assessment, after which it can be decided whether to proceed with diagnostic anaesthesia or diagnostic imaging. The specificity and potential pitfalls of both perineural and intra-synovial anaesthesia should be considered for correct interpretation of the results. Conventional diagnostic imaging modalities include radiography and ultrasound. Most osseous changes can be identified by radiography, however its two-dimensional nature may limit its ability to detect and localise certain abnormalities. The first choice for imaging soft tissue structures is ultrasonography, which enables assessment of most tendons and ligaments in the fetlock region. Contrast radiography can also help detect certain soft tissue lesions in the fetlock region. If lameness is localised to the fetlock region, and radiographic and ultrasonographic findings are unremarkable, or there are several abnormalities that may contribute to the lameness, advanced diagnostic imaging modalities such as magnetic resonance imaging, computed tomography or scintigraphy should be considered. All of these modalities are becoming more accessible to many horses and their owners and examinations can be performed in the standing sedated horse at an increasing number of clinics. In selected cases, diagnostic arthroscopy can also provide useful information. Each clinical case requires an individual assessment. The choice of diagnostic tools should be tailored to the result of the lameness investigation, the budget of the owner and to the prospective treatment and rehabilitation methods available for the horse.
