Empleo de benzodiacepinas en pacientes con depresión tratados en centros de salud mental.

- Abstract
La mayoría de los protocolos terapéuticos de la depresión destacan el tratamiento con antidepresivos y recomiendan limitar al máximo el empleo de benzodiacepinas, especialmente en los pacientes ancianos. Se dispone, sin embargo, de poca información sobre los patrones de uso de las benzodiacepinas en los centros especializados en salud mental. Los autores examinaron el empleo de benzodiacepinas en una muestra extensa de pacientes con depresión tratados en los centros de salud mental de U. S. Department of Veterans Affairs (VA). Método: Se utilize el VA National Registry for Depression para identificar a los pacientes tratados por depresión en las unidades especializadas de salud mental de 129 centros VA durante los primeros 3 meses del año fiscal 2001 (1 de octubre a 31 de diciembre de 2000) (n = 128. 029). Se utilizó la base de datos farmacéutica VA para identificar a los pacientes ambulatorios a los que se había prescrito una receta de benzodiacepinas. Los autores comunican la prevalencia y las pautas de empleo de benzodiacepinas por parte de los pacientes ambulatorios durante el año de estudio (ano fiscal 2001) y evalúan los factores clínicos y demográficos relacionados con el uso de dichas sustancias. Resultados: Durante el año de estudio, al 36 % de los pacientes con depresión se le prescribió una receta de benzodiacepinas, y al 89 %, una receta de antidepresivos. La mayoría de los consumidores (78 %) recibieron una provisión de benzodiazepinas para > 90 días, y el 61 % para > 180 días. Los pacientes ancianos, de raza blanca, de origen hispano y los pacientes con trastornos de ansiedad comorbidos, tenían una probabilidad mayor de usar benzodiacepinas. Los pacientes ancianos también tenían una probabilidad mayor de recibir benzodiacepinas para > 90 días frente a los pacientes más jóvenes, aunque recibieron dosis mas bajas. El empleo de benzodiacepinas vario de forma significativa según la zona del país y el centro. Conclusiones: Los pacientes con depresión tratados en unidades de salud mental recibían, habitualmente, un tratamiento prolongado con benzodiacepinas junto con antidepresivos, una pauta de tratamiento que no se ajusta a las recomendaciones de los protocolos terapéuticos. Los pacientes ancianos tienen mayor probabilidad de recibir benzodiacepinas. Se requieren estudios que examinen la eficacia de esta práctica tan extendida.
