Safety and blood loss in spinal surgery for haemophiliacs: Case series of Japanese haemophiliacs.

- Abstract
The number of spinal surgeries (SS) has steadily and markedly increased in Japan and many countries since the early 2000s.1-3 This is also true for haemophiliacs. Orthopaedic surgery for haemophiliacs is challenging since these patients often need careful management (including the administration of concentrates of various coagulation factors) and sometimes massive blood transfusion. The present case series suggests that the majority of SS procedures can be conducted safely in haemophiliac patients under appropriate coagulation factor replacement therapy. For haemophiliacs scheduled to undergo major surgery, the World Federation of Haemophilia guidelines recommend peak FVIII activity levels between 80% and 100% just before surgery, 60% and 80% on postoperative day (POD) 1-3, 40% and 60% on POD 4-6, and 30% and 50% on POD 7-14.
