- Abstract
The article discusses the findings of the European Trial on Optical Coherence Tomography Optimized Bifurcation Event Reduction (OCTOBER), which aimed to test the superiority of routine optical coherence tomography (OCT) guidance in percutaneous coronary intervention (PCI) of complex coronary bifurcation lesions compared to angiographic guidance. The trial enrolled patients with stable angina or non-ST-segment elevation acute coronary syndrome requiring PCI of a true bifurcation lesion. The primary endpoint was a composite of major adverse cardiac events (MACE), and the results showed that OCT-guided PCI resulted in a lower occurrence of MACE compared to angiography-guided PCI. However, the trial had some limitations, and further research is needed to understand the specific steps of OCT guidance that lead to favorable outcomes.
