ALTERAÇÕES HEMATOLÓGICAS EM PACIENTES CANINOS POSITIVOS PARA ANAPLASMA PLATYS - ESTUDO RETROSPECTIVO.

- Abstract
As hemoparasitoses, doenças transmitidas por carrapatos, em especial Rhipicephalus sanguineus, são consideradas endêmicas no território nacional, dentre elas, a Anaplasmose, que tem como agente etiológico a Anaplasma platys, bactéria pertencente à ordem Rickettsiales e família Anaplasmataceae, que se multiplica principalmente em plaquetas. Os sinais clínicos incluem febre, diarreia, anorexia, perda de peso, petéquias, equimoses, esplenomegalia e alterações laboratoriais como trombocitopenia e anemia. O objetivo deste trabalho foi avaliar as alterações hematológicas presentes em pacientes diagnosticados positivos para A. platys, oriundos da rotina de um hospital veterinário, no período de 5 meses. Foram avaliadas amostras de 5 pacientes suspeitos de anaplasmose, ou por apresentarem inclusões plaquetárias sugestivas de A. platys ou pacientes que apresentaram sinais clínicos compatíveis com a patologia, demonstrando alterações hematológicas pertinentes. Os resultados foram confirmados por reação em cadeia de polimerase (PCR) convencional utilizando primers específicos (SANTOS, 2009). Dos pacientes avaliados, 60% (n=3) não apresentaram alteração no eritrograma e em 40% (n=2) foi observada anemia: uma de caráter regenerativo, com contagem de reticulócitos acima do valor de referência, além de anisocitose, policromasia, Corpúsculos de HowellJolly e Corpúsculos de Heinz na microscopia, e uma anemia não regenerativa, sem reticulocitose ou observações relevantes. Respectivamente, apresentaram hematócrito de 25% e 29% (VR: 37%-54%). O leucograma se apresentava dentro dos valores de referência em 60% (n=3) dos cães, enquanto que 20% (n=1) demonstrava leucocitose por eosinofilia e 20% (n=1), apresentava leucopenia, com presença de linfócitos reativos e monócitos ativados. No plaquetograma, 100% (n=5) dos pacientes apresentava trombocitopenia, com valores variando de 50.000/µl a 183.000/µl (VR: 200.000 – 500.000/µl). A anemia pode estar relacionada a uma displasia transitória da medula óssea ou a uma redução da proliferação de precursores da hematopoiese, já a trombocitopenia pode ocorrer devido a diminuição da meia-vida das plaquetas, além de sua disfunção e destruição imunomediada. Nos esfregaços sanguíneos de 60% (n=3) dos pacientes, foi evidenciada a presença de macroplaquetas e em 40% (n=2) havia presença de inclusões intraplaquetárias sugestivas de A. platys. Como as hemoparasitoses são doenças subdiagnosticadas em cães, torna-se importante o diagnóstico confirmatório de anaplasmose, em decorrência de características clínicas e hematológicas similares a outros agentes patogênicos.
