- Abstract
Con il termine di carico parziale ci si riferisce alla percentuale di peso del paziente che non deve essere superata sull'arto leso per favorire la guarigione dei tessuti e un più rapido recupero delle ADL, senza sollecitare prematuramente le strutture coinvolte nell'intervento chirurgico. Gli studi dimostrano che non tutti i pazienti possono rispettare questa prescrizione. Questo studio mira a dimostrare l'efficacia di un dispositivo semplice ed autoprodotto di biofeedback tattile, definito dagli autori "Incentivatore di Sensibilità al Carico" (ISaC) per il rispetto del carico. Sono stati analizzati 31 pazienti di età compresa tra 18 e 85 anni (M/W:10/21), assegnati in modo casuale ad un gruppo sperimentale e ad un gruppo di controllo. Sono state effettuate due misurazioni con solette digitali «GeBiom» all'inizio (T0) e dopo 15 giorni (T1) per valutare il carico. Al tempo T0, nel gruppo di studio, sono state eseguite due registrazioni, con (T0s) e senza (T0) ISaC. Il dolore percepito e la resistenza alla deambulazione sono stati misurati utilizzando le scale NRS e 6MWT. Tra le due sessioni di misurazione, entrambi i gruppi sono stati sottoposti a sessioni di fisioterapia. Al gruppo di studio è stato richiesto di utilizzare il sistema ISaC durante le attività in piedi durante il periodo di degenza ospedaliera. I risultati intra e intergruppi sono stati valutati con il test di Wilcoxon e il test t di Student. Nessuna differenza é stata riscontrata fra i due gruppi a T0 per il peso dei pazienti e per il ΔPeso (definito come la differenza tra il carico prescritto e quello effettivamente sopportato). L'uso di ISaC a T0 porta un miglioramento immediato nel controllo del carico. Al gruppo sperimentale in T1 vi è minore variabilità al carico impresso rispetto al gruppo di controllo. In entrambi i gruppi si osservano miglioramenti sia alla scala NRS che al 6MWT. Il gruppo di studio mostra una maggiore capacità nell'apprendimento (T0/T0s/T1) e nel mantenimento (T0s/T1) del carico prescritto, giustificando il suo utilizzo per la gestione del carico post-operatorio. Il gruppo di controllo non mostra alcun apprendimento ma aumenta il carico in base alla sensazione soggettiva di fiducia. Non si osserva alcuna differenza intergruppo nel dolore percepito o nella resistenza alla deambulazione.
