Huge endometrioma mimicking ovarian malignancy: report of three cases, what radiology should considered?

- Abstract
Endometrioma is a cystic lesion containing endometrial tissue and it most often develops as a cyst which are most commonly occurred in the ovary. Huge and widespread endometrioma may be misdiagnosed as ovarian malignancy by imaging. Computed tomography (CT) scan had little role in diagnosing endometrioma due to its relatively low sensitivity and specificity. CT scan was performed in 3 women, age 58, 43, and 40 years old which was previously diagnosed as ovarian cyst with suspicion of malignancy based on ultrasound findings, which was then proven to be endometrioma by histopathology examination. Common findings on CT scan included multi loculated, septated cystic mass, value of Hounsfield unit 78, 57, 31 and cancer antigen 125 (CA125) 19.6, 214.0, 141.7 U/mL with symptoms enlarged stomach and abdominal pain. The appearance of huge endometrioma on CT scan should be considered even in older patients and increased CA125 with the same appearances as ovarian malignancy.
