Impact of perioperative use of vasopressors in free flap surgery: A trial sequential analysis.

- Abstract
A recent study published in the journal Head & Neck examined the impact of using vasopressors during free flap surgery in head and neck surgery. The study found that managing intraoperative hypotension with vasopressors did not increase the risk of complications or necrosis of the flap. In fact, the use of vasopressors was associated with a reduced risk of total complications and flap complications. However, there was an increased risk of microvascular thrombosis with intermittent boluses of vasopressors. The study's findings were supported by a retrospective analysis of 777 free flap surgery patients. While the meta-analysis conducted in the study was well-conducted, the authors noted that the robustness of the findings remains uncertain due to the limited number of studies included and the heterogeneity in study design. Trial sequential analyses were conducted to provide additional information, and the results confirmed the robustness of the meta-analysis findings for flap failure, total complications, and flap complications. However, further research is needed to explore the impact of vasopressors on major medical complications in this type of surgery.
