The article presents a case study of a 16-year-old boy with multiple episodes of nephrolithiasis. Plain abdominal film at first presentation revealed a small staghorn calculus obstructing the ureter and multiple stones within the right renal pelvis. The right kidney of the patient was found to be atrophic with 1.6% split function.
