- Abstract
Tendinopatias são consideradas a principal causa de aposentadoria precoce de cavalos atletas. Nesse sentido, a busca por métodos que aperfeiçoem o diagnóstico precoce de lesões tendíneas, ainda quando as alterações não atingiram caráter degenerativo, é cada vez mais desejável. Novas técnicas têm sido desenvolvidas e aplicadas na medicina veterinária; entre elas, pode-se destacar a elastografia. O objetivo deste estudo foi avaliar se a técnica de elastografia ARFI (Acoustic Radiation Force Impulse) seria capaz de detectar alterações na rigidez dos tendões flexores digitais superficiais de equinos hígidos e com lesões induzidas. Foram utilizados oito equinos, cinco machos e três fêmeas, com idade média de 11 ± 4,2 anos e peso de 311 ± 26 kg. No membro torácico esquerdo, induziuse lesão no tendão flexor digital superficial (TFDS), via aplicação única de 2,5 mg de colagenase na porção média do TFDS. A aplicação foi realizada com o cavalo sob sedação com cloridrato de detomidina na dose de 0,01 mg/kg, por via intravenosa, e anestesia local por meio do bloqueio perineural dos nervos digitais palmares lateral e medial no terço proximal da região metacarpiana. O membro torácico direito foi utilizado como controle. Foi utilizado o software Virtual Touch Tissue Quantification e transdutor padrão de 9,0 MHz para obtenção das imagens. O exame ultrassonográfico dos membros torácicos dos equinos permitiu a visualização das estruturas tendíneas e ligamentosas, tanto na avaliação ultrassonográfica convencional (modo B) como para elastografia ARFI. Uma única indução de lesão com 2,5 mg de colagenase foi suficiente para produzir lesão tendínea em todos os animais. Foram observados sinais clínicos de processo inflamatório, tais como aumento de temperatura, aumento de volume e sensibilidade dolorosa locais. A face palmar do TFDS, no ponto de aplicação da colagenase, tornou-se espessada e os animais apresentaram claudicação visível ao trote. Aos elastogramas, no membro controle, os tendões avaliados apresentaram-se não deformáveis, verificandose tecidos com coloração alaranjada, ou tendendo ao vermelho (maior rigidez), e as estruturas adjacentes (tecidos moles) com tonalidades esverdeadas a azuis, indicando menor rigidez tecidual. Setenta e duas horas após a indução da lesão no membro torácico esquerdo foi possível notar nas imagens ultrassonográficas lesões hipoecóicas ou anecóicas, com presença de líquido e edema inflamatório peritendíneo. No elastograma, a região central do tendão flexor digital superficial, principalmente em região 2B e 3A, apresentava-se esverdeada e com diminuição significativa nos valores das velocidades de cisalhamento, indicando diminuição na rigidez tecidual. A elastografia ARFI mostrou-se capaz de identificar o ponto de lesão tendínea induzido experimentalmente, sendo que esse exame revelou possuir 75,8% de sensibilidade e 92,03% de especificidade para detecção de lesões tendíneas no tendão flexor digital superficial de equinos. A elastografia ARFI se mostrou uma técnica ultrassonográfica promissora para o diagnóstico preciso e objetivo de lesões tendíneas em equinos.
