The main causes of microcytic anemia include iron deficiency and thalassemia.[1] Furthermore, a peripheral blood smear revealed a "fish cell" as well as numerous target cells (Figure 1). An index of <13 suggests the presence of thalassemia.[2] Detailed questioning of the patient revealed a history of thalassemia.
