- Abstract
The presence of oxygen is the basis for proper functioning of aerobic biological systems, but all aerobic processes lead to the production of reactive oxygen species. Under physiological conditions, cells are able to prevent harmful effects of free radicals through their natural antioxidant defense systems. Free radicals also exert beneficials effects exemplified by their contribution to ovarial hormone secretion, ovulation and implantation. During normal pregnancy, oxygen and nutrient supply to the fetus is maintained following the ever increasing need of the growing fetus. However, increasing fetal metabolism is associated with increasing free radical formation in both the placental and fetal tissue. In order to counteract these oxidant effects, oxidant/antioxidant balance need to be maintained by increased activity of antioxidant systems. During placentation, the connection between maternal and fetal circulation grows stronger leading to placental accumulation of ROS and placental OS. These induce enhanced local production of antioxidants by cytotrophoblasts and villosus stroma cells. During normal fetal development, growing oxidant production is adequately counterbalanced by increasing activity of antioxidant defense. Fetal hypoxia is the most important stimulus for placental ROS production leading to both placental OS and long-term epigenetic effects through direct binding of free radicals to DNA base pairs. In order to prevent or treat clinical conditions associated with placental OS such as preeclampsia oral supplementation of antioxidants may be necessary, but their optimal use is inadequately defined warranting further studies and the establishment of consensus based guidelines.
