Las dermatosis específicas del embarazo revisadas y reclasificadas: resultados de un estudio retrospectivo en dos centros de 505 pacientes embarazadas.

- Abstract
Antecedentes: las dermatosis específicas del embarazo representan un grupo heterogéneo de enfermedades en esta etapa. Con excepción del penfigoide gestacional, la patogénesis es desconocida y existe muy poca bibliografía relacionada con sus características clínicas. Se conoce poco sobre ellas, ya que son escasas y la terminología es confusa. Existen dos clasificaciones: la primera, propuesta por Holmes y Black en 1983, incluye el penfigoide gestacional, la erupción polimórfica del embarazo, el prurigo del embarazo y la foliculitis prurítica del embarazo; y la segunda, propuesta por Shornick en 1998, comprende al penfigoide gestacional, la erupción polimórfica del embarazo, el prurigo del embarazo y la colestasis intrahepática del embarazo. Él sugiere que la foliculitis prurítica del embarazo se incluya en el prurigo del embarazo. Objetivos: investigar la frecuencia y características clínicas de las dermatosis pruriginosas del embarazo y proponer una clasificación racional. Material y métodos: se realizó un estudio retrospectivo en dos hospitales dermatológicos universitarios desde 1994 hasta el 2004. Se revisaron 596 mujeres embarazadas (420 de Londres y 176 de Graz) y se obtuvieron los datos de 505; se excluyeron 95 pacientes sin enfermedad prurítica y 9 por información incompleta. Resultados: la edad promedio fue de 30 años. El 44% fue primigrávida, 99% con neonato único y 76% en el primer trimestre. Las pacientes tuvieron dermatosis en todo el cuerpo, incluidas las palmas y plantas; todas eran atópicas y 20% tenían antecedente. En 80% aparecieron durante el embarazo. Los diagnósticos incluyeron eccema del embarazo (49.7%), erupción polimórfica del embarazo (21.6%), penfigoide gestacional (4.2%), colestasis intrahepática del embarazo (3%), prurigo del embarazo (0.8%), foliculitits del embarazo (0.2%) y enfermedades misceláneas de la piel (20.6%). El eccema del embarazo, el prurigo y la foliculitis fueron las afecciones más frecuentes y se resumieron con erupción atópica del embarazo. La erupción polimórfica del embarazo, el penfigoide gestacional y la colestasis intrahepática del embarazo aparecieron en el tercer trimestre, pero la erupción atópica se manifestó mucho antes. Las pacientes primigestas y con embarazos múltiples tuvieron, principalmente, erupción polimórfica del embarazo. Hubo afección abdominal en aquellas con erupción polimórfica del embarazo, penfigoide gestacional y antecedentes de colestasis intrahepática. Conclusiones: es importante clasificar las dermatosis del embarazo. La inmunofluorescencia y los estudios de laboratorio son característicos para establecer el diagnóstico de penfigoide y colestasis intrahepática del embarazo; las características clínicas ayudan a distinguir entre erupción polimórfica y erupción atópica del embarazo. La principal limitante del estudio fue su modalidad retrospectiva.
