Voriconazole compared with liposomal amphotericin B for empirical antifungal therapy in patients with neutropenia and persistent fever.

- Abstract
El objetivo de este articulo era evaluar si voriconazol es tan eficaz como anfotericina B liposomal, y menos tóxico, en el tratamiento empírico de los pacientes neutropénicos con fiebre persistente. Para ello se realizó un ensayo clínico aleatorizado donde participaron un total de 73 centros distribuidos en los EE.UU., Canadá, India y Europa desde marzo de 1998 a septiembre de 1999. Se incluyeron un total de 837 pacientes (415 en la rama de voriconazol y 422 en la rama de anfotericina B liposomal). Se evaluó la respuesta al tratamiento. La respuesta global fue del 26% con voriconazol y 30,6% con anfotericina B liposomal. La respuesta fue independiente de la administración de profilaxis antifúngica y del uso o no de factores estimulantes de colonias. Había menos infecciones fúngicas documentadas en los pacientes tratados con voriconazol que en aquellos tratados con anfotericina B liposomal (8 [1,9 por ciento] vs. 21 [5,0 por ciento], p = 0,02). La toxicidad fue menor en el grupo tratado con voriconazol, había menos casos de reacciones graves relacionadas con la infusión (P <0,01) y de nefrotoxicidad (p <0,001). Sin embargo, la incidencia de hepatotoxicidad fue similar en los dos grupos. Los pacientes que recibieron voriconazol presentaron más episodios de cambios visuales transitorios que los que recibieron anfotericina B liposomal (22 por ciento frente a 1 por ciento, p <0,001) y más alucinaciones (4,3 por ciento frente a 0,5 por ciento, p <0,001). En este estudio se habían incluido pacientes con más de 12 años que habían recibido tratamiento previo con quimioterapia por leucemia, linfoma u otros cánceres, o trasplante con células progenitoras hematopoyéticas. Se iniciaba tratamiento antifúngico empírico cuando el paciente llevaba menos de 96 horas con antibióticos sistémicos por fiebre y neutropenia (< 500 neutrófilos/µl durante 96 h y < 250 neutrófilos/µl en las 24 h anteriores a la asignación aleatoria). No se incluían pacientes que habían tenido previamente infección fúngica invasora (IFI) o presentaban alteraciones de las pruebas de función hepática. Los pacientes se aleatorizaban a voriconazol o anfotericina B liposomal. El tratamiento se continuó hasta 3 días después de la recuperación de los neutrófilos (> 250 neutrófilos/µl) o hasta un máximo de 12 semanas en los enfermos con IFI documentada. Asignación aleatoria. Se realizó estratificación según: a) el grado de riesgo de infección fúngica (alto y moderado); b) el uso o no uso de terapia antifúngica profiláctica sistémica, y c) el centro participante. Fueron definidos como pacientes de alto riesgo: trasplante alogénico de células madre hematopoyéticas y los que hubieran recibido quimioterapia por recidiva de leucemia. El resto fue clasificado como pacientes con riesgo moderado. Enmascaramiento: no. Un comité independiente, que no conocía el tratamiento asignado, revisó y clasificó todas las infecciones fúngicas probables o probadas, usando un protocolo previamente definido. Objetivo. Valoración de no inferioridad del voriconazol con respecto a la anfotericina B liposomal. La eficacia fue definida como: 1. No desarrollo de una nueva IFI. 2. Supervivencia a los 7 días del final del tratamiento. 3. No retirada prematura del tratamiento. 4. Desaparición de la fiebre durante el período de neutropenia. 5. Respuesta satisfactoria al tratamiento de cualquier infección fúngica presente al comienzo del estudio. Las conclusiones de los autores deben valorarse con precaución. En el diseño del estudio, los autores predefinieron que el valor de "eficacia no inferior" del voriconazol con respecto a la anfotericina B liposomal sería un porcentaje de éxitos para el desenlace combinado no inferior al 10%. Como el intervalo de confianza inferior de la respuesta favorable (10,6%) sobrepasó ese límite, no se debe considerar probada la "eficacia no inferior" del voriconazol. Evaluación de los resultados y de las conclusiones. No desarrollo de una nueva infección fúngica invasora: 98,1% en el grupo voriconazol y el 95% en el grupo anfotericina B liposomal. Supervivencia a los 7 días del final del tratamiento: 92,0% en el grupo voriconazol y 94,1% en el grupo anfotericina B liposomal. No retirada prematura del tratamiento: 90,1% en el grupo voriconazol y 93,4% en el grupo anfotericina B liposomal. Desaparición de la fiebre durante el período de neutropenia: 32,5% en el grupo voriconazol y 36,5% en el grupo anfotericina B liposomal. Respuesta satisfactoria al tratamiento de cualquier infección fúngica presente al comienzo del estudio: 6/13 en el grupo voriconazol y 4/6 en el grupo anfotericina B liposomal. La Food and Drug Administration (FDA) rechazo la indicación de voriconazol para el tratamiento de los enfermos neutropénicos con fiebre persistente, aunque ha autorizado su uso para algunas infecciones fúngicas, como la aspergilosis invasora.
