- Abstract
SUMMARY A seven-week-old girl presented with recurrent apnoeic episodes and respiratory insufficiency. The child was hypotonic, weak, areflexic and had a paradoxical movement of the right hemidiaphragm. Cranial nerve and sphincter function was normal; there was no fasciculation. Nerve conduction studies showed a severe axonal sensory and motor peripheral neuropathy. Biopsy of the sural nerve revealed marked axonal atrophy. The infant is now over two years of age, is ventilator-dependent and has clinical and electrophysiological evidence of further progression of one of the earliest reported presentations of a congenital peripheral neuropathy. RÉSUMÉ Neuropathie périphérique congénitale se présenrant sous forme d'apnée et d'insuffisance respiratoire Des épisodes apnéiques récidivants et une insuffisance respiratoire furent observés chez une petite fille de sept semaines. L'enfant était hypotonique. faible, aréflexique et présentait un mouvement paradoxal de l'hémidiaphragme droit. Les nerfs crǎniens et les fonctions sphinctériennes étaient normaux; il n'y avait pas de fasciculation. Les études de conduction nerveuse révélèrent une neuropathie périphérique axonale sensori-motrice grave. La biopsie du nerf saphene externe révéla une atrophie axonale accentuée. La fillette, ǎgée maintenant de deux ans, est en ventilation assistée présente des signes cliniques et électrophysiologiques d'évolution dans la plus précoce des neuropathies périphériques congénitales décrites. ZUSAMMENFASSUNG Congenitale periphere Neuropathie mit den klinischen Symptomen Apnoe und respiratoriscite Insuffizienz Ein sieben Wochen altes Mädchen wurde mil rekurrierenden Apnoen und respiratorischer lnsuffizienz vorgestellt. Das Kind war hypoton, schwach und ohne Reflexe und hatte paradoxe Bewegungen der rechten Zwerchfellhälfte. Hirnnerven- und Sphinkterfunktion waren normal; es fanden sich keine Faszikultationen. Untersuchungen der Nervenleitgeschwindigkeit zeigten eine schwere axonale sensorische und rnotorische periphere Neuropathie. Die Biopsie des N. suralis ergab eine ausgepragte axonale Atrophie. Das Kind ist jetzt über zwei Jahre alt, beatmungsabhangig und hat klinisch und elektrophysiologisch Hinweise fur eine Progression der congenitalen peripheren Neuropathie, die eine der jüngsten in der Literatur beschriebenen ist. El estudio de la conducción nerviosa mostró una neuropatía periférica axonal sensitiva y nerviosa grave. La biopsia del nervio sural mostró una atrofia axonal marcada. La nina tiene ahora más de dos años de edad, es respirador dependiente y muestra evidencia clinica e electromiográfica de un curso ulterior progresivo. Es uno de 10s casos más precoces publicados de una neuropatia periférica congénita.
