- Abstract
Introducao e objetivos: A Hipomineralizacao molar-incisivo (MIH) constitui defeito qualitativo do esmalte dentario que acomete 1 a 4 primeiros molares permanentes, podendo afetar incisivos permanentes. A condicao representa problema de saude publica por estar relacionada a carie dental e sua etiologia permanece inconclusiva. Comunidades quilombolas sao grupos etnico-raciais que vivem em situacao de vulnerabilidade social, ma nutricao e dificuldade de acesso a servicos de saude, fatores que podem predispor ao desenvolvimento da MIH. Este estudo observacional transversal teve por objetivo determinar a prevalencia e os fatores associados a MIH em criancas e adolescentes quilombolas rurais do nordeste brasileiro. Materiais e metodos: A amostra foi constituida por criancas e adolescentes residentes na comunidade quilombola rural Lagoas em Sao Raimundo Nonato, Piaui (populacao censitaria). Foram incluidos os individuos com idade de 8 a 14 anos, que possuiam as coroas dos quatro primeiros molares permanentes totalmente visiveis. Foram excluidos do estudo criancas e adolescentes que apresentaram fluorose dentaria em grau moderado/avancado, hipoplasia do esmalte ou amelogenese imperfeita, bem como os individuos que se encontravam em tratamento ortodontico fixo no momento da avaliacao, condicoes que dificultariam o diagnostico da condicao. Foram coletados dados sociodemograficos e relacionados a saude pre, peri e pos-natal e realizado exame clinico dentario, por quatro examinadores previamente treinados e calibrados (Kappa.0,8) para diagnostico da MIH. Os dados foram digitados duplamente e analisados pelo programa StatisticalPackage for the Social Science (SPSS), versao 20.0. Foram realizadas analise descritiva dos dados e regressao de Poisson com variancia robusta. A magnitude da associacao foi avaliada por razao de prevalencia bruta e ajustada (RP), e intervalos de confianca de 95% (IC 95%). Resultados: A amostra foi constituida por 251 criancas e adolescentes que atenderam aos criterios de inclusao da pesquisa (populacao censitaria). A prevalencia de MIH foi de 46,6%. No modelo final, a presenca de MIH esteve associada a diabetes gestacional (RP = 2,49; IC95% 2,08-2,99; p<0,001) e a sofrimento fetal agudo (RP = 1,39; IC95% 1,04-1,86; p=0,025). Conclusao: Conclui-se que a prevalencia de MIH na populacao estudada foi alta e associada com diabetes gestacional e sofrimento fetal agudo.
