- Abstract
Stroke is one of the most disabling diseases, often leading to neurological disability and even death. It is considered a medical emergency and need urgent recanalization. Citicoline is approved in some countries for the treatment of stroke based on its mechanism of action. The aim of this clinical trial is to assess the effect of citicoline on cerebral hemodynamics and vasoreactivity using Transcranial Doppler and functional recovery in acute ischemic stroke patients. Clinical trial involved 60 acute ischemic stroke patients, divided into two groups: treatment and control groups (30 patients for each). We tested cerebral hemodynamics and vasoreactivity by using Transcranial Doppler (TCD) whereas assessment of functional status was done through modified Rankin scale (mRs). Measurements were achieved at time of presentation and after 8 weeks, then they are compared with the previous readings and between the two groups.Regarding cerebral hemodynamics, there was statistically significant increase in peak systolic velocity, end diastolic velocity, mean flow velocity and insignificant decrease in pulsatility index and resistance index in treatment group compared to controls; vasoreactivity tests showed statistically significant increase in breath holding index in treatment group compared to controls. Patients prescribed citicoline showed statistically significant improvement in functional status when compared to controlswhen mRS was applied. Citicoline improved cerebral hemodynamics, vasoreactivity parameters and functional recovery in acute ischemic stroke patients.
