ALTERAÇÕES HEMATOLÓGICAS DE CÃES COM DIAGNÓSTICO CITOLÓGICO DE Ehrlichia/Anaplasma spp. ATENDIDOS EM UM HOSPITAL UNIVERSITÁRIO VETERINÁRIO.

- Abstract
Os gêneros Ehrlichia e Anaplasma compreendem bactérias intracelulares obrigatórias, transmitidas por vetores invertebrados. Objetivou-se avaliar as alterações hematológicas em cães com diagnóstico citológico de Ehrlichia/Anaplasma spp. atendidos em um hospital universitário veterinário. Foi realizado um levantamento de dados dos hemogramas de cães atendidos entre outubro de 2018 e outubro de 2019, que apresentaram inclusões de Ehrlichia/Anaplasma spp. em células mononucleares e/ou em plaquetas na hematoscopia de esfregaços sanguíneos. O estudo foi aprovado no CEUA n° 2920261219. Os dados hematimetria (He), hemoglobina (Hb), volume globular (VG), volume globular médio (VGM), concentração de hemoglobina globular média (CHGM), índice de anisocitose (RDW), plaquetometria (P), índice de anisocitose plaquetária (PDW) e volume plaquetário médio (MPV) foram analisados. Um total de 17 cães foi positivo. Os valores médios encontrados e os valores de referência utilizados foram respectivamente: He (5,34 x 106/µL / 5,50 a 8,50 x 106/µL), Hb (10,6 g/dL / 12,0 a 18,0 g/dL), VG (26,16%/37-55%), CHGM (34,85%/31-35%), VGM (62,17 fL/60-77 fL), RDW (13,86%/8,94-12,41%), PDW (15,56 fL/10,66-15,05 fL), MPV (8,02 fL/7,52-16,36 fL) e P (78.500/µL / 200 a 700 x 10?/µL). A maioria dos animais (94,1%) apresentou anemia, sendo normocítica (64,7%) normocrômica (88,2%) principalmente. Entretanto, 70,6% apresentaram anisocitose, que pode estar associada à presença de macrócitos (eritropoiese acelerada), micrócitos (deficiência de ferro, insuficiência hepática e esferocitose) ou ambos. Interessante observar que apesar do mecanismo de anemia induzida por Ehrlichia/Anaplasma spp. ser por imunossupressão da medula óssea, estes agentes podem induzir hemólise imunomediada, devendo sempre ser avaliada a hematoscopia em animais positivos. Observou-se trombocitopenia em 88,2% dos animais, podendo estar associado à destruição imunomediada, consumo ou sequestro. Em 80% dos cães o MPV não apresentou alteração. Entretanto, o PDW esteve aumentado em 93,3% dos animais, indicando aumentada atividade medular. Este índice tem maior sensibilidade a variações de tamanho das plaquetas, aumentando antes dos valores de MPV serem alterados. Concluiu-se que anemia normocítica normocrômica, trombocitopenia, anisocitose eritrocitária e plaquetária foram achados frequentes em animais com diagnóstico citológico de Ehrlichia/Anaplasma spp. Sugere-se que a avaliação do RDW e PDW seja adicionada aos perfis de avaliação de animais com esta hemoparasitose.
