Severe congenital neutropenia: A comprehensive case report.

- Abstract
Introduction: Congenital cyclic neutropenia syndrome, a constitutional genetic disorder, is marked by significantly low neutrophil counts. Those affected often experience early-life neutropenia, recurrent fever, mouth ulcers, gingivitis, and heightened susceptibility to recurring infections, posing a heightened risk. Case Report: A 7-year-old male patient reported to the Department with a chief complaint of pain in his left lower back tooth region for 1 month. The patient also had a history of recurrent oral ulcers on the tongue for 7 months of age. Past medical history revealed that the patient was under medication from 7 months of age until 1 year back due to vitamin C deficiency. During the first dental visit, extraction of 84 was performed as the patient presented with dehiscence in 84 and was kept under review. The patient reported back after 2 weeks with oral ulceration. The patient was referred to the department of pediatrics to rule out systemic problems. As the patient had recurrent ulceration, he was further referred to the department of hematology, where a diagnosis of severe congenital neutropenia was given after gene mapping. Clinical significance: The report emphasizes the importance of recognizing uncommon sources of immunosuppression in children with recurrent oral ulcers and insufficient dental care. Equipping healthcare professionals with the necessary knowledge is crucial for treating similar patients effectively. It also mitigates the risks of morbidity and mortality from neutropenic sepsis.
