INTRODUCTION: The lack of laxity in the skull region , holds a real difficulty in managing the lost of complex tissue. Method of reconstruction in this area, depends on local characteristics as; etiology, location, size, depth of the defect; but also on patient general health status. SUBJECT: We present a case, involving immediate cranial reconstruction of a fronto-parietal scalp defect, after removal of exposed, infected cranioplasty material. MATERIAL: Male patient presented an old cranial reconstruction in fronto-parietal region after a traffic accident, was referred to our service, with exposure and infection of the cranioplasty material. METHOD: Complete removal of the metallic material including all the surrounding necrotic tissues, leads to a large defect. Reconstruction was performed with two fasciocutaneous local flaps, rotated and advanced. DISCUSSIONS: For this patient, we choose a simple procedure for reparing a large scalp defect. The surgical time allocated to this procedure was short with minimum local morbidity and a good esthetic result. A complete healing was obtained in a short period of time. CONCLUSION: This one stage surgical procedure, was an adequate method for reconstruction of soft tissues coverage in this region, characterized by lack of skin laxity/ elasticity, and in presence of many posttraumatic scars.
