- Abstract
Introducción: En ONWARDS 1, un ensayo abierto y aleatorizado de fase 3a (NCT04460885), en pacientes con Diabetes mellitus tipo 2 (T2D) sin tratamiento previo con insulina. Objetivo: Evaluar el tiempo en rango (TIR), el tiempo por debajo del rango (TBR) y el tiempo por encima del rango (TAR), la duración de hipoglucemia en las semanas 0-4, 22-26, 48-52 y 74-78 bajo tratamiento con insulina icodec una vez a la semana o insulina glargina U100 una vez al día. Material y Métodos: TIR (70-180 mg/dL), TAR (>180 mg/dL), TBR (<70 y <54 mg/dL), la duración media de hipoglucemia <70 mg/dL y la proporción de episodios gastados <54 mg/dl fueron evaluados con un sistema de monitorización continua de glucosa (MCG) doble ciego en periodos preespecificados en las semanas 0-4, 22-26, 48-52 y 74-78. Resultados: En las semanas 22-26, 48-52 y 74-78, la TIR media con icodec cumplió el objetivo recomendado de >70%; TIR y TAR mejoraron significativamente con icodec frente a glargina, sin diferencias significativas entre los brazos en TBR <54 mg/dL (Tabla). Hubo una diferencia a favor de la insulina glargina en TBR <70 mg/dL en las semanas 48-52 y 74-78, pero la media de TBR <70 mg/dL y TBR <54 mg/dL estuvieron por debajo de los objetivos recomendados (4% y 1%, respectivamente) en todos los periodos de tiempo para ambos brazos. TIR, TBR y TAR no se modificaron significativamente entre los brazos en las semanas 0-4. La duración media de la hipoglucemia <70 mg/dL y la proporción de tiempo <54 mg/dL fueron similares entre los brazos en todos los periodos de tiempo. Conclusiones: En resumen, TIR y TAR en las semanas 22-26, 48-52 y 74-78 mejoraron significativamente con icodec frente a glargina U100, sin diferencias significativas en TBR <54 mg/dL y una duración similar de la hipoglucemia <70 mg/dL entre los brazos.
