V73: Laparoscopic Seminal Vesical Sparing Radical Prostatectomy Using Low Energy Source: Better Continence and Potency.

- Abstract
Introduction: After the pioneering period when only few teams were performing Laparoscopic Radical Prostatectomy, Laparoscopic approach to Radical Prostatectomy has become widespread with several technical variations. We report our experience of seminal vesical sparing Laparoscopic Radical Prostatectomy. We also studied the use of low energy source especially in the region of prostatic apex and neurovascular bundle and evaluate its outcome on continence and potency. Materials and Methods: Between Nov 2003 and Nov 2007, 28 patients aged 50 to 77 years underwent laparoscopic radical prostatectomy with vesico urethral anastomosis. Patients were followed up every 3 months. The 12 patients operated in later part of our experience underwent seminal vesical sparing Laparoscopic Radical Prostatectomy between April 2007 and November 2007 using minimal energy source. Results: The mean follow-up was 14 months. 11 of 16 patients in group I were continent as compared to 14 of 16 patients in group II by the end of 12 months. Of the 8 patients who were potent preoperatively in group I, 4 remained potent 3 month after LRP in group II, 9 patients of 16 were potent preoperatively and 5 remained potent 3 months after LRP. Conclusions: Overall the current operative, oncological and functional results of laparoscopic radical prostatectomy appear to approximate those of open radical retropubic prostatectomy. Use of low energy source at bladder neck and neurovascular bundle and also sparing seminal vesical is associated with better continence and potency. Nevertheless, longer follow-up and more mature data are needed definitively to establish the fact.
