- Abstract
Originally, laryngology procedures were office-based but then transitioned to the operating room with the successful introduction of general anesthesia.[1] In-office procedures in laryngology have been used increasingly to diagnose and treat laryngeal and airway disease. We use a flexible distal-chip nasolaryngoscope with a 2.0-mm-minimum working channel (Olympus America, Center Valley, PA) and 25-gauge, 200-cm injection-therapy needle catheter (Boston Scientific, Marlborough, MA) for this technique. We have found that 83% of patients who undergo SILSI will do well long-term without needing endoscopic surgery.[3] Hoffman et al. found that a similar proportion of patients (14 out of 17, 82%) have not needed or requested operative treatment since beginning SILSI.[2] SILSI leaves all other treatment options viable. SILSI is a relatively simple, well-tolerated treatment that allows for precise injection of subglottic stenosis under direct visualization in most patients.
