Le paludisme d'importation à Strasbourg: une étude épidémiologique, clinique, biologique et thérapeutique.

Résumé Nous avons mené une enquête rétrospective portant sur 210 sujets hospitalisés de 1984 à 1995, ayant tous présenté un accès de paludisme, confirmé par la présence de trophozoïtes sur le frottis ou la goutte épaisse. Nous avons analysé les données épidémiologiques, cliniques, biologiques et thérapeutiques. Le nombre de cas annuels augmente progressivement, avec un maximum atteint en 1995. Il existe des variations saisonnières correspondant à l'augmentation des voyages dans les pays d'endémie, en janvier et d'août à octobre. On met en évidence une majorité de contamination en provenance d'Afrique. Dans ce continent, l'espèce la plus fréquente est Plasmodium falciparum. L'âge moyen des patients est de 33 ans. Les manifestations cliniques sont souvent polymorphes; les signes les plus fréquents sont: fièvre, frissons, sueurs et céphalées. Sur les 210 cas, nous avons recensé 15 formes graves sclon la définition de l'OMS avec deux accès pernicieux à proprement parler, tous d'évolution favorable après la mise en route du traitement. Nous avons observé 5 cas de paludisme chez des femmes enceintes. Deux d'entre elles ont fait un accès grave, dont un accès pernicieux typique. Parmi les anomalies biologiques marquantes, on note: la thrombopénie, les stigmates d'hémolyse, l'hypocholestérolémie et l'hypertriglycéridémie. On ne connait pas la signification de ces deux derniers éléments, malgré leur grande fréquence. L'hypertriglycéridémie semble plutôt associée aux formes graves de paludisme. L'analyse de la prophylaxie montre que la majeure partie des accès est en rapport avec une prophylaxie absente, mal prise, ou inadaptée au pays visité. Ceci confirme l'importance de l'information aux voyageurs. Le traitement des accès évolue en fonction de l'arrivée sur le marché des nouveaux produits qui tour à tour voient émerger de nouvelles résistances. Parmi ces traitements, la quinine reste la référence thérapeutique. Summary In a retrospective study, we registered 210 patients hospitalized in Strasbourg for malaria from 1984 to 1995. The diagnosis was always confirmed by presence of the parasite on blood smears. We analysed the epidemiological, clinical, biological and therapeutic data. The number of cases rose each year, with a maximum in 1995. The majority of cases occured in January and from August to October, these periods corresponding with the return of travelers. In most cases, infection took place in Africa. In this region, Plasmodium falciparum is the most frequent species of the parasite. The mean age of the patients was 33 years. The clinical manifestations were polymorphic: fever, chills, sweating, and headache were very frequent. We noted 15 serious infections (with the WHO's definition) and two cases of cerebral malaria. All cases had a favorable outcome. Five cases occured in pregnant women; two of them had a severe form of malria. Among the biological abnormalities, we found thrombopenia, haemolysis, hypocholesterolaemia and hypertriglyceridaemia. The significance of disturbance of the lipid metabolism is not known. Hypocholesterolaemia is very frequent, and hypertriglyceridaemia seems to be associated with severe malaria. Most malaria attacks occured in patients without adequate chemoprophylaxis. This confirms the importance of prophylactic information given to patients by their physicians. Resistance develops against each new medication that is available; among these quinine remains the drug of choice to cure severe malaria.
