Susan Shapiro's book, "Speaking for the Dying: Life‐and‐Death Decisions in Intensive Care," explores the role of health care surrogates in making medical decisions for older Americans who are unable to do so themselves. The book focuses on two intensive care units in a large urban hospital and reveals that advance directives, which are meant to guide surrogates in decision-making, often have little impact. Medical personnel and even family members frequently ignore or override these directives. The study highlights a significant gap between the law and its implementation in the ICU, raising important questions about the effectiveness of current legal protections for patients.
