- Abstract
Nail involvement can be seen in 3-15% patients with lichen planus (LP), with permanent nail dystrophy occurring in approximately 4% patients.1 Additionally, isolated nail LP may be seen in 1-10% cases.2 LP preferentially affects the nail matrix where it progresses slowly with potential scarring; therefore, early diagnosis is important. Clinical features of all 20 nails were meticulously recorded as changes attributable to nail matrix, nail bed, or nail fold disease. Clinical findings Clinically, nail matrix involvement was seen in 40.2% nails, whereas nail fold involvement was seen in 33.8% nails. There was a significant difference with onychoscopy being able to detect a greater number of nail changes as compared to clinical examination with regard to longitudinal melanonychia, onychoschizia, true leukonychia, erythematous lunula (both homogeneous and stippled erythema), distorted lunula, nail bed erythema, and splinter haemorrhage ( I P i < 0.05).
