Chronic intussusception in children caused by Ascaris lumbricoides.

- Abstract
Chronische Intussuszeption (CI) definiert man als eine Intussuszeption, die 14 Tage, oder länger, anhält. Die Diagnose ist oft verzögert, da die klinischen Manifestationen nicht spezifisch sind. Die Symptome sind zumeist intermittierende Bauchschmerzen und manchmal ein palpabler Tumor im Bauch. Zusätzlich besteht ein deutlicher Gewichtverlust. Die Diagnose basiert auf typischen Merkmalen im Ultraschall oder auf Ergebnissen der Röntgen Untersuchungen mit Bariumeinlauf beziehungsweise der CT (Computertomographie) Untersuchungen. Die Therapie ist chirurgisch, mit obligatorischem Ausschluss von möglichen zugrundeliegenden Läsionen. Wir berichten über ein 31/2 jähriges Mädchen mit chronischer Intussuszeption. Das Blutbild zeigte eine Eisen-Mangel-Anämie. Die Faeces wurde mehrfach positiv auf Ascaris lumbricoides getestet. Eine antiparasitäre Therapie mit Mebendazol wurde wiederholt durchgeführt und brachte anfangs Besserung. Nach Beendigung der Therapie traten die Symptome allerdings wieder auf. Beim darauf folgenden chirurgischen Eingriff wurde eine chronische Intussuszeption der Regio ileocolica gefunden, die bis zur Flexura hepatica reichte. Postoperative Kontrolluntersuchungen zeigten komplette Heilung. Wir glauben, dass die Infestation mit Ascaris lumbricoides ein ätiologischer Faktor für die Entwicklung der chronischen Intussuszeption bei diesem Kind war. Chronic intussusception (CI) is defined as an intussusception lasting for 14 days or more. Because the clinical manifestations are non-specific, the diagnosis is usually delayed. Symptoms include intermittent abdominal pain, sometimes an abdominal mass is palpable and there is a marked weight loss. Diagnosis is based on typical features revealed by ultrasound or barium enema and CT investigations. Therapy is surgical with obligatory exclusion of possible underlying lesions. We report a case of a 3.5-year-old girl with chronic intussusception. Laboratory blood findings revealed sideropenic anemia and stools positive for Ascaris lumbricoides. Anti-parasitic therapy with mebendazole was repeatedly administered with early improvements but soon after symptoms relapsed. During surgery an ileocolic chronic intussusception extending to hepatic flexure was found. Postoperative control examinations proved complete healing. We suspected that Ascaris lumbricoides infestation was an etiologic factor for the development of chronic intussusception in the child.
