Remoção intraoral de sialólito em equino sob sedação e anestesia local.

- Abstract
Sialólitos são concreções compostas por núcleo orgânico e camadas periféricas de carbonato de cálcio, que se desenvolvem no interior do ducto ou de uma glândula salivar. A etiologia mais comum parece ser a obstrução do ducto por corpo estranho de origem vegetal. Cavalos com sobrecrescimento dentário são predispostos ao acúmulo de alimento entre as pontas excessivas de esmalte e o vestíbulo, visando evitar lesão dos tecidos moles. Esta retenção de alimento próximo ao óstio bucal facilita a migração de corpo estranho para o interior do ducto parotídeo e possível formação do sialólito. O sinal clínico mais comum é a presença de massa firme, não aderida, próxima à extremidade rostral da crista facial. Quando provoca obstrução completa, pode levar à distensão do ducto parotídeo, febre, dor à palpação, disfagia, anorexia e presença de exsudato purulento na cavidade oral. O diagnóstico se dá pela apresentação clínica, mas pode ser auxiliado pelo exame radiográfico ou ultrassonografia. O tratamento é baseado na remoção por massagem, quando as formações são pequenas, ou excisão cirúrgica. Um equino, macho, Puro Sangue Lusitano, de 24 anos, foi atendido apresentando aumento de volume no vestíbulo direito havia 18 anos. O exame físico revelou aumento de volume bem delimitado, oval, firme, não aderido, sem edema ou calor local e sem dor à palpação. O exame radiográfico evidenciou presença de corpo estranho radiopaco ovalado que se estendia do dente 107 ao 109, confirmando o diagnóstico de sialolitíase. Optou-se pela excisão cirúrgica pelo acesso intraoral com o paciente em posição quadrupedal. Após sedação com detomidina (0,02 mg/kg) e morfina (0,1 mg/kg), e bloqueio local infiltrativo com lidocaína a 3%, realizou-se incisão de 4 cm sobre a formação. Utilizouse, então, uma pinça Allis para apreender os fragmentos do sialólito, enquanto a formação era pressionada externamente contra a incisão. Prescreveram-se antiobioticoterapia, anti-inflamatório não esteroidal e lavagem da cavidade oral com água corrente. No 20° dia pósoperatório, havia grande quantidade de alimento no interior da ferida e do ducto parotídeo. Realizou-se, então, a aproximação dos bordos da ferida em padrão simples separado utilizando Nylon 0. No 27° dia, outra avaliação intraoral revelou completa cicatrização e ausência de alimento no ducto parotídeo. Apesar da sialolitíase ser de fácil identificação clínica, o exame radiográfico contribuiu para a eliminação dos diagnósticos diferenciais. O tamanho da formação obrigava a remoção cirúrgica como forma de tratamento. Optou-se pelo acesso intraoral pelo fato do sialólito se encontrar na porção distal do ducto parotídeo, além de minimizar o risco de fístulas salivares externas. Em concordância com autores que relataram o acesso intraoral, priorizou-se a cicatrização por segunda intenção. A entrada de alimento facilitada pela extensão da ferida impediu a cicatrização. Optou-se, então, pela primeira intenção tardia, mantendo a ferida coaptada, impedindo a entrada de alimento e permitindo a completa cicatrização. Apesar da complicação pós-operatória, o resultado obtido demonstra que o procedimento cirúrgico intraoral pode ser realizado de maneira efetiva em equinos em posição quadrupedal sob sedação e anestesia local.
