- Abstract
Introduction: Prostate sarcoma, a rare malignant neoplasm that originates from the connective tissue of the prostate, represents a complex area of investigation and treatment within urological oncology. The prevalence of this type of cancer is low compared to prostate cancer, but its clinical evolution is often more aggressive and challenging. Understanding the clinical evolution of prostate sarcoma and developing effective surgical approaches are essential to improve therapeutic options and outcomes for affected patients. Objective: The objective of this systematic literature review is to comprehensively examine the clinical evolution of prostate sarcoma and evaluate the results and approaches of surgical treatment through a critical and integrative analysis of scientific articles, clinical studies and other relevant sources. Methodology: This systematic literature review followed PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyzes) guidelines. Searches were conducted in the PubMed, Scielo and Web of Science databases to identify published studies. The following descriptors were used in the search: "prostate sarcoma", "clinical evolution", "surgical treatment", "clinical results" and "therapeutic approaches". Results: 15 articles were selected. Analysis of scientific studies and articles revealed that prostate sarcoma is a heterogeneous neoplasm with significant histological and clinical variations. Its clinical evolution often shows aggressive behavior, characterized by rapid tumor growth, high rate of metastasis, and resistance to conventional treatments. Early diagnosis is challenging due to its rarity and the overlap of symptoms with other prostate conditions. As regards surgical treatment, the reviewed studies revealed a multidisciplinary approach as being the most effective. Radical surgical resection of the prostate and surrounding tissues is often required to achieve disease-free margins. However, surgery may be complicated due to the anatomical location of the prostate and the spread of the tumor. Post-surgical reconstruction and rehabilitation are key parts of the therapeutic process. Conclusion: This systematic literature review highlighted the complexity of the clinical evolution of prostate sarcoma and the crucial importance of surgical treatment in the management of this rare and challenging condition. The multidisciplinary approach and the search for innovative surgical approaches are essential for improving the clinical results and the quality of life of the patients affected by this neoplasia. As new research emerges and therapeutic approaches evolve, it is critical to continue to investigate and develop effective strategies to address prostate sarcoma comprehensively and successfully.
