- Abstract
The article describes the case of a 58-year old man with previous inferior myocardial infarction who was referred for cardiac resynchronization therapy pacemaker for decreased left ventricular ejection fraction and left bundle branch block. The patient's electrophysiology result confirmed the diagnosis of ventricular tachycardia, while activation mapping revealed His-Purkinje conduction system (HPS). The importance of the width of QRS rhythm in the differential diagnosis is noted.
