Atresia esofágica tipo III: cierre primario con brecha mayor a dos cuerpos vertebrales (anastomosis con técnica de no tocar): reporte de dos casos.

- Abstract
Introducción. La atresia esofágica tipo III es la anomalía congénita más frecuente (1 de cada 3 000 nacidos vivos), se asocia a prematurez y polihidramnios. La cirugía convencional consiste en realizar cierre de fístula y anastomosis término terminal, siendo el factor limitante para ésta la distancia entre ambos cabos. En el pasado cuando esta distancia era mayor a 2 cuerpos vertebrales la técnica recomendada era cierre de la fístula, esofagostomía, gastrostomía y en un segundo tiempo sustitución esofágica mediante ascenso gástrico o interposición colónica. Se propone realizar anastomosis primaria en todos los casos de atresia de esófago tipo III, realizando 3 puntos técnicos fundamentales: 1. Abordaje extrapleural, 2. Movilización del cabo proximal y 3. Anastomosis con técnica de "no tocar los bordes". Objetivo: evitar sustituciones esofágicas en las atresias de esófago tipo III. Casos clínicos. Caso 1. Femenino 35 semanas, antecedente de polihidramnios, nace por cesárea, peso 2 300 g, Apgar 8-9, sialorrea abundante, el estudio con medio hidrosoluble mostró atresia de esófago tipo III, con 2.5 cuerpos vertebrales de distancia hasta la carina. Se sometió a cirugía realizando anastomosis, evolucionando sin complicaciones. Esofagograma a los 7 días y 5 meses mostró anastomosis permeable, estenosis esofágica distal congénita. El paciente actualmente asintomático. Caso 2. Masculino obtenido por parto vaginal, sin antecedentes. Peso 2 550 g, Apgar 8-9, sialorrea, imposibilidad para el paso de la sonda, el estudio con contraste mostró atresia de esófago tipo III con casi 3 cuerpos vertebrales de distancia a la carina; se sometió a cirugía realizándose anastomosis término terminal, con evolución favorable; esofagograma de control al mes mostró estenosis esofágica de 90%, que ameritó dilatación con balón hidrostático en 2 ocasiones. Actualmente asintomático. Conclusiones. En las atresias de esófago tipo III es factible realizar el cierre primario aun en los casos de grandes brechas entre los cabos esofágicos (2 cuerpos vertebrales o más), utilizando los principios de: 1. Abordaje extrapleural. 2. Movilización del cabo proximal esofágico (ganando de 1 a 1.5 cm de longitud).
