- Abstract
This retrospective cohort study investigated the risk of cardiovascular disease events in women with a history of gestational hypertension and preeclampsia. Women who had at least 1 singleton live birth in England in 1997–2015 were included in the analysis. Among 2,359,386 first live births, there were 85,277 and 74,542 hospital admissions for gestational hypertension and preeclampsia, respectively. During 18 years of follow-up, the incidence of total cardiovascular disease events for normotensive women, women with prior gestational hypertension and women with prior preeclampsia were 57.1 per 100,000 person-years, 85.8 per 100,000 person-years, and 99.3 per 100,000 person-years, respectively. Adjusted HRs for total cardiovascular disease events were 1.45 (95% CI 1.33–1.59) in women with prior gestational hypertension and 1.62 (95% CI 1.48–1.78) in women with prior preeclampsia. Gestational hypertension was strongly associated with future dilated cardiomyopathy and unstable angina, whereas preeclampsia was strongly associated with hypertrophic cardiomyopathy and acute MI.
