- Abstract
Background: Atrial septal defect(ASD) is a common congenital heart disease. Large ASDs result in increased pulmonary flow and pressures resulting in Eisenmenger syndrome(ES). Sacubitril/Valsartan is an angiotensin-receptor neprilysin inhibitor (ARNI), multiple Randomised control trials(RCT) have demonstrated its effectiveness in reducing heart failure(HF), rehospitalization, improving left ventricular ejection fraction (LVEF), LV reverse remodeling and lowering mortality in HF patients. Aim: The aim of this study is to evaluate right ventricular function in patients with large atrial septal defects with Eisenmenger syndrome initiated on ARNI. Methodology: In this single center prospective study, all ASD patients with ES and HF prescribed ARNI between August 2019 to August 2022 were identified. The NYHA functional class, 6MWT, TAPSE, LVEF, RVEF, PA pressures, NT-proBNP for each patient was established at baseline and at the end of 12 months. Results: The comparison of changes of the NYHA class, 6MWT, TAPSE, RVEF, PA pressures and NT-pro BNP from baseline to 12 months was statistically significant in our study. (p=0.001). Conclusion: ARNI treatment improves functional class, 6MWT, RVEF, PA pressures and decreases heart failure hospitalizations and NT-pro BNP in ASD-ES HF patients.
