- Abstract
About 3.5 tonnes of chemicals were emitted from a reaction vessel on the phosgenation plant at International Biosynthetics Ltd (IBIS), Widnes, Cheshire, UK in December 1991. Wind blew the vapour cloud some 4 km and affected about 60 people, 35 of whom received hospital treatment. All were released after observation, but there were concerns about site safety and the emergency response. The company was fined £4000 for breaches of the Health and Safety at Work Act 1974 Sections 2 and 3 relating to the health, safety, welfare and exposure to risk, of employees.
